New Patient Registration

registration1The doctors welcome new patients who live within our practice area. 

We have an open list policy and welcome requests for registration from patients living or moving into our practice area. If you are unsure of our boundaries please speak to a receptionist. We will ask you to complete a new patient registration form (GMS I) and make an appointment for a new patient check.

If you move out of the area, we regret that you must register with another practice.

From April 2015 in line with Government recommendations we are required to provide all of our patients with a Named GP who will have overall responsibility for your care and support. However this does not prevent you from seeing any GP of your choice when appointments are available.
